Risk Factors & Blood pressure greater than 135/85 mmHg:
* History of Heart Failure
* History of angina, coronary artery bypass grafting (CABG), heart attack, stents for heart disease, peripheral vascular disease (PVD), stroke, mini-stroke (TIA)
* Familial hypercholesterolaemia
* Carotid narrowing
* Smoking
* Type 2 Diabetes
* Gestational diabetes
* HbA1c 41–49 mmol/mol
* BMI more than 30, or waist circumference ≥ 102 cm in males or ≥ 88 cm in females
* Chronic Kidney Disease
* New atrial fibrillation
* Family history of death from heart attack or stroke at less than 50 years in a first-degree relative
* Diabetes in a first-degree relative
